This post is quite scary.  This may put us in denial (say it is not true) or be very very scared because we are in the Philippines.  If the disease will spread from the Pangasinan, we will be the first to be hit. We have friends and relatives from Pangasinan.  If it is true, how do we deal with it (as the two predictions made by prophet Sadhu in April 2013 in the Philippines at the 24th National Prayer Gathering in Cuneta Astrodome Pasay City.  His prediction on typhoon Yolanda, Bohol earthquake, Palawan floods were true.

He predicted that a terrible disease will emanate from Pangasinan, and eventually spread to Cebu.  It will be characterized by decay of flesh that will spread to the bones.

If it is true:

1.  How will our health officers deal with the sick?

2.  How about the dead?

3.  Is our country again unprepared for this (as in Typhoon Yolanda) for this eventuality?

Sterile Insect Technique for control of tsetse flies in Africa

In l969, while in college, I remember making a paper in the Biology class regarding tsetse flies which causes trypanasomas, which is called Nagana in cattle, and sleeping sickness in man. This is an epidemic in Africa. This has caused starvation, and destruction of destruction of human resources due to death it has caused.

This disease infest 36 countries in Africa, and covers an area between 9 to 10 million square km.  And causes damage between $600 to $ 1 billion in death of livestock.  This gives truth to the saying health is wealth

Today, I learned from Al Jazeera, that up to now there has been no effective cure/prevention of the disease.

What are they doing?  They intend to eradicate the vector, the tse tse flies by spreading irradiated male tse tse flies.  It is hoped that the flies will be eradicated by 2016.  Scientists however are not sure as to its effect on the fragile African ecosystem

Asi Tava good for prostate ailment? A story

I met Ven again two days ago and he told me story why his prostate has not been bothering him again.  He got this story from Mr. Liit Gragera who lived for 4 years in USA.  He went home because of a prostate condition that doctors told him in USA needed an operation.

He also learned that Asi Tava is good for prostate.  So he chewed the leaves (4 per day) juiced it, used it in salad.

Surprise.   He  did not have to be operated on.  And 10 years has passed. So he is suggesting to elder males over 50 to try Asi Tava to have healthy prostate

No to privatization - is this simply a way to preserve jobs and tenure?

Leo Obligar in his program Kanta ng Bayan criticized the government plan to privatize 39 public hospitals.  He cited this after noting that many young poor children are dying from cancer.   And where will this poor children get the resources to get cure.  Naturally they cant be treated for a song in private hospital.

One wonders if the consolidation of hospitals by the MVP group resulted in improvement of health care in the Philippines.  One thing for sure, this resulted in more expensive health care, albeit the delivery was better and more efficient.

But accessibility and affordability is becoming a big challenge

Dr David Shetty, a philanthropist and cardiac surgeon, was earlier mentioned in this post by Dr. Mercola as basis of comparison between heart operation in India and USA.  It would cost about $100,000 in USA, for an indigent about l/100 of that amount.  In the March 14, 2013 issue of Time Magazine, David Brill explained why health care costs in US are high.

 He founded the Narayana Health  in 2001(formerly Narayana Hudayalayana) He founded the hospital because he can not find an employer who buys into his vision.  The heart surgery is the largest in the world with 1,000 beds and does 30 heart surgeries a day.   He has performed l5,000 heart surgeries (can you beat that?)  Fast Company has classified Narayana as one of the 50 innovative companies in the world

How can Dr. Shetty do it:

There are 3 foundations for this capacity of Dr. Shetty to cure many Indians (now about a billion, one of the most populous countries in the world)

1.  Economies of scale.  This is employed by large manufacturing companies and retailers.  Large volume can bring down costs, and exerts a tremendous force on the supplier to lower costs.  Thus cost of procedures in Shetty hospitals is much much lower than either govt or private hospital in India.

2.  Desk for  assisting the needy.

    1.  Working with  foundation such as Heart Foundation that assists patients who need heart bypass or other other expensive heart operations;

    2.  Assisting patients obtain funding/assistance from govt agencies or foundations that help the needy.  The patients need not worry about the money aspect, or turned down for treatment just because he does not have money.  All that patient's family have to worry about is how to make the patient well with their care.

   3.  Cross subsidy  -  60% of the patients of Dr Shetty pay the full amount, 20% pay premium because they can afford to pay;  10% pay a little, 10% are indigent and pay nothing.  The 80% who pay can shoulder the 20% who pay a little or nothing.

He was personal physician of Mother Teresa of Calcutta and did a heart surgery on her.  

   The Aravind hospital is another hospital that offer low cost cataract operation in India, organized by Dr. V.  (He is now dead)

  I hope that we can have this type of hospital in the Philippines.  Many large hospitals and many those that are being put for profit alone.

In the Philippines, it is noted that with the emergence of  world class hospitals:   SLMC, and TMC, the cost of medical care shot up.  The consolidation of hospitals by MVP group:   Riverside, Cardinal Santos, MMC was hoped to have brought economies of scale, but it did not.  The cost of health care merely went up.  Much of medical care in the Phil, amounting to more than P300 billion is 2011, came from out of the pocket.

  If the Lord wills it, I hope I can organize something like that of Dr. Shetty's hospital.  I hope I can muster enough number of former MBAH students who are MDs who are social entrepreneurs who can help me put a hospital just like what Dr. Shetty has put up.  Perhaps Dr. Vega  of Davao Medical Center can share his expertise in running a 2,000 bed hospital and putting up a heart and kidney center, a Director of NCMH, the current Usec and former congresswoman Dr. Garin?

Or can we just ask Dr. Shetty move in the Philippines.  So far he has made his group climb up to 30,000 beds.  He predicts that there will be shortage of health care personnel world wide.  The problem is how to make standardize procedures to lower costs.

The WHO reports alarming  increase of  incidence of cancer. The report took 5 years to complete by 250 scientists.   8.2 million died of cancer in 2012.  1 out 5 men will have cancer before the age of 75.  1 out of 6 women will develop  cancer. Lung, liver, stomach, colorectal and breast cancers cause the most cancer deaths each year.  With this incidence, 22 million will die from cancer.  60% of the incidence of cancer occur in Africa, Asia, Central and South America.   30% of cancer is preventable.

The focus now is prevention rather than cure.  (Often, even early detection does not cure cancer;  cancer can be unstoppable once it gets into the patient)

Cancer can be caused by lifestyle, and cured by changing lifestyle.




Often caused by:

1.  Tobacco,

2.  Alcohol,

3. Obesity,

4.  Viral infection as HPV.

5.  lack of exercise;

6.  Low input of vegetables and fruits

Cancer is a generic term for a large group of diseases that can affect any part of the body. Other terms used are malignant tumours and neoplasms. One defining feature of cancer is the rapid creation of abnormal cells that grow beyond their usual boundaries, and which can then invade adjoining parts of the body and spread to other organs. This process is referred to as metastasis. Metastases are the major cause of death from cancer.

The problem

Cancer is a leading cause of death worldwide, accounting for 8.2 million deaths in 2012 (1). The main types of cancer are:
  • lung (1.59 million deaths)
  • liver (745 000 deaths)
  • stomach (723 000 deaths)
  • colorectal (694 000 deaths)
  • breast (521 000 deaths)
  • oesophageal cancer (400 000 deaths) (1).

What causes cancer?

Cancer arises from one single cell. The transformation from a normal cell into a tumour cell is a multistage process, typically a progression from a pre-cancerous lesion to malignant tumours. These changes are the result of the interaction between a person's genetic factors and three categories of external agents, including:
  • physical carcinogens, such as ultraviolet and ionizing radiation;
  • chemical carcinogens, such as asbestos, components of tobacco smoke, aflatoxin (a food contaminant) and arsenic (a drinking water contaminant); and
  • biological carcinogens, such as infections from certain viruses, bacteria or parasites.
WHO, through its cancer research agency, International Agency for Research on Cancer (IARC), maintains a classification of cancer causing agents.
Ageing is another fundamental factor for the development of cancer. The incidence of cancer rises dramatically with age, most likely due to a build up of risks for specific cancers that increase with age. The overall risk accumulation is combined with the tendency for cellular repair mechanisms to be less effective as a person grows older.

Risk factors for cancers

Tobacco use, alcohol use, unhealthy diet and physical inactivity are the main cancer risk factors worldwide. Chronic infections from hepatitis B (HBV), hepatitis C virus (HCV) and some types of Human Papilloma Virus (HPV) are leading risk factors for cancer in low- and middle-income countries. Cervical cancer, which is caused by HPV, is a leading cause of cancer death among women in low-income countries.

How can the burden of cancer be reduced?

Knowledge about the causes of cancer, and interventions to prevent and manage the disease is extensive. Cancer can be reduced and controlled by implementing evidence-based strategies for cancer prevention, early detection of cancer and management of patients with cancer. Many cancers have a high chance of cure if detected early and treated adequately.

Modifying and avoiding risk factors

More than 30% of cancer deaths could be prevented by modifying or avoiding key risk factors, including:
  • tobacco use
  • being overweight or obese
  • unhealthy diet with low fruit and vegetable intake
  • lack of physical activity
  • alcohol use
  • sexually transmitted HPV-infection
  • urban air pollution
  • indoor smoke from household use of solid fuels.
Tobacco use is the single most important risk factor for cancer causing about 22% of global cancer deaths and about 71% of global lung cancer deaths. In many low-income countries, up to 20% of cancer deaths are due to infection by HBV and HPV.

Prevention strategies

  • Increase avoidance of the risk factors listed above.
  • Vaccinate against human papilloma virus (HPV) and hepatitis B virus (HBV).
  • Control occupational hazards.
  • Reduce exposure to sunlight.

Early detection

Cancer mortality can be reduced if cases are detected and treated early. There are two components of early detection efforts:
Early diagnosis
The awareness of early signs and symptoms (for cancer types such as cervical, breast colorectal and oral) in order to get them diagnosed and treated early before the disease becomes advanced. Early diagnosis programmes are particularly relevant in low-resource settings where the majority of patients are diagnosed in very late stages and where there is no screening.
Screening
Screening is defined as the systematic application of a test in an asymptomatic population. It aims to identify individuals with abnormalities suggestive of a specific cancer or pre-cancer and refer them promptly for diagnosis and treatment. Screening programmes are especially effective for frequent cancer types for which a cost-effective, affordable, acceptable and accessible screening test is available to the majority of the population at risk.
Examples of screening methods are:
  • visual inspection with acetic acid (VIA) for cervical cancer in low-resource settings;
  • PAP test for cervical cancer in middle- and high-income settings;
  • mammography screening for breast cancer in high-income settings.
